READER SURVEY GUIDELINES
For this project to be successful it is vitally important to have timely responses from a sufficient number of readers. This requires an attractive incentive in the form of a prize drawing, gift, charitable contribution or anything that will ensure reader response.
Incentives & Drawing
You are responsible for all costs associated with prizes and incentives, and for delivery of prizes to winners.
The incentive should have universal appeal to attract a broad base of respondents and should be of sufficient value to ensure an adequate number of respondents. A contribution to charity for each response often makes a difference for many readers who do not normally participate in prize drawings.
Verified will select winners who will be jointly notified by you and Verified.
Promotion
You should promote the survey on the front page and wherever possible in the publication. If an online version is offered, promote it on your website (a link provided by Verified). If available, provide Verified with your list of registered visitors for an email blast (no additional charge).
Approval of the Questionnaire
The following items on the questionnaire need to be customized. Please do not alter any other portions of the artwork provided by Verified.
- Logo
- Publisher's Message, Name
- Prize and/or Charity Information
- Publication Name (Question #8,9)
- Publication URL (Question #6)
- URL if online survey is offered (link provided by Verified)
- Response Deadline (30 days following publication date)
- Prize Drawing Date (45 days following publication date)
Completed artwork must be submitted to Verified prior to insertion into your publication.
Please submit final artwork to Verified at least 10 days prior to run date for approval.
Posting the online survey
Once your survey has been approved, the rest is up to Verified and your readers.
Printing requirements for the in-pub survey
You are responsible for printing of the survey questionnaire.
The survey must be printed on cardstock. USPS recommends .01" - .016" thickness. Card size must be exactly as per the supplied artwork. Use artwork supplied by us to align the return address artwork precisely, complying with USPS requirements.
Insertion
Randomly insert the questionnaires into at least 20,000 papers or use your full print run. Insert it in one issue only.
Postage and Tabulation
Our price includes postage and tabulation of up to 400 responses. Any additional responses will be invoiced at $.63 for postage (USPS cost). Optionally, an additional $.95 will be charged for each survey over 400 responses to be tabulated and included in the survey findings.
Deliverables
After completion of the survey, you will be provided with an attractive summary report and the data tables generated from the survey tabulation. Reprints of the summary report are available at an additional cost.
